What is an Ishikawa diagram?

An Ishikawa, cause and effect, or fishbone diagram organizes possible causes into clear groups. It opens lines of inquiry. A cause written on the diagram remains a hypothesis until evidence supports it.

Fact

Information supported by a photograph, document, measurement or reliable account.

Hypothesis

A possible explanation that needs evidence.

Supported finding

A conclusion consistent with the available evidence, with uncertainty recorded.
